Ballet Class Registration

To speed up the process of registering your child for a class...

1) Please download and print out the registration form below.

2) Please ONLY sign, date and fill in the contact and dance history sections. The other details that need to be filled in will happen on the date of registration or shortly afterwards.

3) Bring the form with your signature, contact info and dance history to the registration on Wednesday, Sept. 9th, 2009 from 4:00 to 6:00 at the Studio.

4)To guarantee your space in a class, it is best to call the academy at (802) 875-2881 prior to registration for further information.
